Default Lying about address at Planned Parenthood

My girlfriend has an appointment on Monday to get an exam for birth control at Planned Parenthood. She's pretty paranoid about her parents finding out, so she wants to lie about her address on the demographic form. Is there a way they can tell? Of course if they check her drivers license, but what if she says she doesn't have one? She might be able to use her school ID instead. Of course it really won't matter because she's going to tell them not to send anything, and I've heard they're pretty honest about sticking to that. I'm going to put my real info down in case they need a way to contact her on a seperate part of the form.
Back to my question... what happens if they find out it's not her real address? Can she use her school ID instead? Like I said, I know they're confidential, it's just for her own reassurance that her parents won't find out.
And while I'm here... anyone have any experience with Ortho Evra (the patch)? We're thinking about trying it but some of the side effects are stronger since there's more exposure to hormones... but it would be very cconvienient since she owuldn't have to worry about hiding a case of pills from her parents and taking a pill at the same time very day.

Tell them your real address, or better yet, get a god damn P.O. box. Also, tell them not to send anything. If they do, you could probably sue them for breach of medical confidentiality, and get civil damages.

True, but you still lose the respect of your conservative as fuck parents.

Fun fact: Because Abstinence-only sex education increases teen pregnancy and STD rates so effectively, the Bush administration has forbidden the CDC from comparing the effectiveness of actual sex ed vs. abstinence only.

You could try the patch or you could get Depo Provera aka The Jab. One injection every three months.... I do know that side effects can be pretty annoying, but there's no harm in trying it.

As for your question, I don't think they're legally allowed to send stuff if you ask specifically not to, and you're over 16.... But that could just be in NZ?
