Pain management therapy
Ok, I guess I will be starting this soon. I told my doctor to give me some marcaine, or lidocaine, or something similar or I would just go online and get it. I said, if I go online and get it, it costs me much more, and is illegal. If you do it, it's cheap, and legal, and I know it's off label usage, but I am always very careful, and VERY sterile, and like I said, I'm doing it anyway, it's the only thing that ever works.
So she says, come see me Monday, and we will discuss PMT.
So, I am under the impression that this means better drugs. I am on hydros now, which don't do SHIT, at ANY dose (though I don't tell her the dosage part) And I was wanting to hear some opinions on PMT, experiences, what to expect, tips, if ya got em.
What the fuck exactly do they do? Also, I've heard sometimes patients are drug tested, which means I can't smoke pot, and in that case FUCK PMT. Anything they do for me short of amputation does not compare in terms of relief, to marijuana.

Re: Pain management therapy
I've never had direct experience with them, but I did look into it some for myself and have known people who did go through it.
From what I know, while they are places where you can get good pain killers, it is usually only after a long process of them trying all sorts of alt way to deal with the pain. They try to use pills only as a last resort.

Re: Pain management therapy
Since you're legitimately looking for pain relief PMT's should be just for you. You said yourself that the lidocaine etc. did better for you than opioids, so if you come in there with that attitude they'll be overjoyed to help someone who isn't just trying to bag a script.
As far as drug testing, I don't think they do unless you fuck up somehow.

That shit(PMT) is a joke....
It's like a legal drug ring.....
take advice from somone who didn't do it, but witnessed the effects first hand, this is NOT the way to deal with whatever pain you may have....
this may sound crazy buuuuuut suck it up. smoke pot. go to the GYM and do any type of exersize that isn't painful or uncomfortable.
just get the endorphins pumping.
Oxycodone in any constant amount is NO JOKE. but u already knew that...

Re: Pain management therapy
Unfortunately, Pain Management does make a habit of drug testing it's patrons. When you start going, visits will be monthly usually and the tests are random but expect the first few visits to be tested everytime. After you have been on the same meds/dose for a while, you will more than likely get bumped to only having to visit the doctor once every two months. This won't happen for at least 6-8 months at the earliest, probably longer though. You should be getting two scripts per visit when it happens though.. One with a "DO NOT FILL UNTIL xx/xx/xx" 30 days after the date on the one that can be filled sooner. Sometimes you can have them mail it to you, or send it directly to the pharmacy of your choice (in the contract/paperwork you sign and they give you they highly recommend you frequent the same pharmacy).
Oh and some Pain Managements have instituted pills counts. So every time you visit the doctor, you have to bring your meds and they better check out, or rapid down dose. If you do fuck up some how and get kicked off, they still have to down dose you, they can't just cut you off. Can be dangerous if they did that and they won't risk a law suit.
My girlfriend could give more details. This is just a condensed version of what she went through. YMMV.
I'm pretty sure as well, if I'm remembering this right, her meds went like this..
Morphine ER > Morphine IR > Methadone 5s > Methadone 10s > Higher daily dose of Methadone 10s where she is at now. sorry, don't remember the morphine dosages. And they switched her form methadone 5s to 10s because, 90 10mg pills is easier on a pharmacy than 180 5mg ones. Her dose is higher now though.
And she is one of the people that does need it. Multiple reconstructive surgeries. Sucks but at least she can manage now.

I'm working on that too. I don't even want the opiates, and I will be GOD DAMNED if those motherfuckers drug test me, or count my fucking pills to MAKE SURE I am getting addicted.
I'm thinking of telling my doctor that I smoke pot, and it helps more than any opiate, and tell her that if marijuana use is going to bar me from PMT, don't even fucking bother. I don't want the opiates, and if they do give them to me, I will use them either to get high, or to make money. What I want is fucking lidocaine, not a drug addiction. I fucking hate the american medical institution.

Re: Pain management therapy
I saw a pain management guy for a few months before I finally just gave in and had surgery, and while I wasn't drug tested, it was the worst experience ever.
I had been getting medication from my GP, and had been worked up to 50ug fent.
When I went in to this douche, he switched me down to 3x30mg ms contin and 6x100mg tramadol a day.
When I said it wasn't working and I was in worse pain, he dropped the morphine to 3x15mg and the tramadol to 1x200mg (but it's XR! ). Yeah, don't spend too much time trying to figure that out.
But on the other hand, there are plenty of people who have success with PMT. Not to speak on Panthrax's behalf, but I think he found more relief once he got into PMT. Or was it ike? I don't remember. One of them.
So like anything, it's hit or miss.
I had an awful experience, others have had fantastic experiences. Fingers crossed you're part of that latter demographic :)
go to the GYM and do any type of exersize that isn't painful or uncomfortable. The man's leg fell into an auger, dude.

Well, if it involves a pain clinic, then they will definitely drug test you, HOWEVER, they usually hook it up with some good opiates...

SN, why don't you just snag lidocain at a walk in clinic or doctors office? I always see tons of the shit lying around. If I had any practical use for it I could have stolen hundreds of bottles by now.

Re: Pain management therapy
As I have told you before, I spent a lot of time with a guy that was run over by a transport truck, he lived with his gimped leg for a few years, and I never ever saw him happy. He was always miserable and in utter pain.
He is my friends dad, and when I would stay over there at night, I would hear him screaming in pain just from trying to sleep. He DIDN'T do PMT, he got his leg amputated and got a morphine script which apparently works wonders for him... it may not for you (as it seems) He also smoked as much pot as he wanted because it really helped for him, and no PMT is going to allow a known cannabis user.
He got done what he knew for a long time would fix him, he had it chopped the fuck off and donated to science. No more than a 6 months after it was amputated, he bought a 16 speed mountain bike and began riding a lot! He also walks his three legged rotty (hilarious) a lot and I have seen the man run, also attempt riding on a skateboard!!!! He fell, needed help up, but was laughing and fine. "You fuckin' kids"
I know for a fact he is happier without his leg, when he had the leg, he became a raging alcoholic, but over time that has changed, and he honestly drinks those .5% grocery store beers!
I think (this is me... well, him also) that if he went for the PMT route, he would have been dicked around with on various meds, had to do tedious painful tasks and he would probably still be drinking.
He IS addicted to morphine, but to be honest, he is so much better off than with the alcohol... the man used to sit in the garage all day watching TV, smoking dope, drinking and eventually causing a terrible scene or hurting himself. Now, he goes on various bike rides, smokes as much weed as he can get his hands on, and takes morphine orally.
I think you know what I am saying, and you have been saying it all along, I think the best route would be to (if the pain is that bad and it looks like the best option) get the fucker amputated, allow doctors to prescribe you what they will (and they WILLLLL!) and if you want to take them, take them, if not... set them aside for a rainy month, or a cash exchange. Then, go ahead and do what you think works the best, smoke weed, take opiates if you want, and if lidocaine really works for you, then by all means, use it... We all know you have the shit to NOT kill yourself or do something when it comes to your pain and the drugs you take that is sketchy.
When it comes down to amputation, they are going to cut off your leg, you spend several days in hospital (maybe longer...?) Then, you go home and manage with it yourself. You take what you need. No one making you do various excersizes, testing you, making you show up at their office once a week.
Though, I have heard some bad shit about fantom pains, and drugs don't fix that... can't medicate an invisible/detached limb.
I wish you the best of luck, and make the best decision for yourself.
When I was working at the dreary grocery store today, I saw all these uptight, stressed out assholes, everyone just seemed like an asshole and was in a huge rush and were very snobby to each other, inconsiderate, you get the picture... I went to go on break and chain smoke 3 smokes and I walked past this older gentleman in a wheel chair, smile accross his face, sitting on his ass all the time, loving his life. I walked past and the guy fucking wheeled his chair out of my way as I walked past and nodded to me and smiled.
I felt rewarded... NO ONE in dreary grocery store #664 EVER so much as looked me in the eye, no one said thank you, no one ever steps out of the way for other people, they are all crashing their carts into each other in a rush, no "oops sorry... *smile*" just on with their shitty day.
And this old guy, who has clearly been dealt a shit hand, NO legs to speak of... someone probably shot his legs off in a war, but he was OK, he didn't seem like a bitter old asshole, he seemed happy.
I bet if he had his old mangled legs hanging off that chair he would have spit on my shoes.
When I saw my friends dad with his leg, he was a douche, when he got it cut off, he felt like... reborn, even read the bible... Then yelled about it... but happily.
I never saw him happy before it came off... and I glad for him that he went through with it and dealt with his shit on his own.

Re: Pain management therapy
I wish that someone gave me the advice White Lightening gave you. I started PMT 1 year ago. I take 90 10mg percs per month; 100 vicodin 10 mg every 25 days and I've had valium forever, every moth 120 10 mg pills.
I can't wait to get off all of it. That stuff eats you up inside.
Good luck,
S

Re: Pain management therapy
Well down here in south florida and i'm sure plenty of other places most "Pain Management Clinics" are basically oxycontin dispensarys. You go in get an mri, xray, etc and then get some kind of pain medicine even if nothing is wrong with you. If you do have real pain then you'll probably get something pretty decent. I haven't actually had any experience going to one of these places but i know about 25-30 who have personally done it and have told me its pretty much a joke.

SN had the lower part of his leg ripped to pieces after stepping into an auger bit... if its taking lidocaine injections to take most of the pain away your in a shit load of pain... and hes looking at a life of waiting for his bones in his leg to fuse together and at that point hes not going to be able to use his leg and be in more pain than ever... you cant really exercise in a situation like that...
SN i would say go for it, and try to get a good supply of maricane going. i imagine its going to take alot of convincing to get them to let you inject it on a daily basis, but if tell them you arent there for pills just to get out of pain and that all you want is some injectable nerve blocking medications that arent abusable at all, they might be a bit more willing...
man go find some god damned insurance and get that leg hacked off, be out of pain, get a prosthetic one, get back in shape and live a generally pain free life...
there is a bottle of injectable lido somewhere in this house, if i could send it to you i would try... even if it could keep you out of pain for a few hours...

Re: Pain management therapy
I don't have the time at the moment to reply to all this, I have a huge reply in mind, but I just wanted to say thank you to all of you, and as for amputation, I am working on that as well. I have to get release forms from workmans comp, from my old doctors, old records from before, take em to an ortho dr I was referred to, he will look at it, see if there is nothing else to be done, then refer me back to the surgeon, who will amputate. Thing is, I have to gather all the paperwork myself, most of it has to be requested in writing and shit, and then once I get all that done, the ortho dr has a fucking three month+ waiting list before I can be seen. I have damn good insurance, it's just red tape fucking bullshit.
More later, got shit to do atm.

SN as for the amputation it is still possible to have phantom pains during limb loss. Just something to think about.

As for phantom pains, they can be easily treated by looking into a mirror, or even using virtual reality. It tricks your brain into thinking the other limb is there and relieves the pain.

Yes, I am well aware of phantom pains, and the various techniques to help it, like phantom itching, just scratch the other leg.
Phantom pains are no concern though, the way I walk hurts the rest of me, so even with phantom pain, I would still be walking right, and be in much much less pain.
And hey jamaica, I would be interested in that, I could probably trade you a test kit or something for it. Like, I pay to ship the test shit, you pay to ship lido. It's only a couple bucks on both sides, so worth the risk.

Re: Pain management therapy
no idea, i use 20mg of methadone a day, sometimes another 10 when I get home. the 20mg is to keep my feet/back from hurting while walking around on concrete all day which I've always had a problem with but cannot get a doctor to give me jackshit for. Works well for me, if you do ever go towards that opiod route let me suggest Methadone, as a addict and someone who uses it to self medicate for pain and opi addiction I must say I find it much better than short acting opiods, and even the ER/Contin types such as OxyCon/ER, MorER, etc.
For example, I took a 80mg teva the other day before work which worked well but I noticed some minor backpain, where as i've never noticed any what so ever with just 20mg of Done...
So thats my 2 cents if you ever go down that route and your doc will play ball with Done, which more and more are starting to instead of using 20/40mg OCs, much cheaper for their patients and to a extent less stigma, plus the fact that it just fucking works better...
If so just watch out for the constipation, its horrible with Done, but at just 20mg doses I find I can go once a day...
Hope you find somehting that works bro, i've heard you speak before on how fucked up you are, no one should live in pain, so for real I reall hope you get some good results on w/e you choose to do...

I don't know any links, only word of mouth stuff, scratching the nub itself, the other leg, slapping the other leg where it itches, the mirror, holding the nub, pretty much anything to make that connection again that it is actually not there.

That's interesting.. the phantom pain stuff (not that its a good thing or that im wishing it upon you or anything) but I wonder what it would feel like. It's definately gotta be because of something in your brain, I wonder if they could come up with a drug that would alter your brain into not trying to send signals to that part of your leg.. or maybe they already have that..

iirc the part of the brain responsible for the sensory input of that leg is suddenly not recieving input, and like a nearby part will take over.

Re: Pain management therapy
So I went to the appt, got there a half hour early, had to take my son with, his mom had a meeting to go to.
So I get in the back of the office, the only one there for that Dr at the time, and SIT IN THE FUCKING ROOM for over an hour. Of course babies get bored, so hes screaming, I'm trying to calm him down, waiting and waiting, and he gets the point where NOTHING is working, he just wants to sleep, and he can't sleep anywhere but his own bed. So he's freaking, I'm getting impatient, well, enraged really, for having to wait so fucking long, being the only one there, so I leave.
I call my wife, hysterical, she calls them, explains whats up, they say, call your pharm, have em fax in your script request, and we will get it in, and then come back wednesday for the appt, arranged for a baby sitter all that.
But the thing is, I ALREADY FUCKING FAXED THAT SHIT. TWO FUCKING WEEKS AGO. I called and called, and had the pharmacy fax in requests, ALL of which were ignored.
The nurse asks me about my meds, are you still on this, that etc. On anything else? Well, yeah, hydrocodone, which for some reason isn't on the fucking chart. WTF.
Why would they say, fax it in, and act like they will give it to me, when they didn't for what is now, almost a month. She left me without pain meds for a month.

YOUR A FUCKING FAGGOT.
the guy is trying to make a logical decision and you are fuckin with his brains.

And hey jamaica, I would be interested in that, I could probably trade you a test kit or something for it. Like, I pay to ship the test shit, you pay to ship lido. It's only a couple bucks on both sides, so worth the risk.
hmm, would have to figure out a way to ensure that my parents didn't go snooping through a random package in the mail for me... its only a small vial of lido and im not even sure how much is left in it...

Regardless of the state of the free healthcare its better than nothing, which is what the majority of Americans have right now, NOTHING... So enjoy your insurances (or I suspect your parents insurance), I'd love to have some but my job sucks balls and doesn't offer any and I cannot get a company to touch me due to my documented medical issues such as diabeties, sleep a-need(sp?), and problems with my addicition that have been documented after a overdose..
Basically I owe a fuckload of doctors, most of the hospitals are pretty nice and have no income programs that pretty much take care of the whole hospital bills, now doctors are a different story, I owe about 12 different docs a shitload of $$, funny thing is is that Ive never even meet most of these docs b/c I was in a coma at the time due to using to many opiates and having full liver failure from it...
Im babbling, but yeah, shitty healthcare is better than non, which is what the majority of us Americans have... So count yourself lucky cowboy.. I need to try to get some damn Medicare

Re: Pain management therapy
Ok, so I finally got in to see the doctor, and she is referring me to PMT, and says drugs are rarely used, and that things have kinda flipped from how they used to be, and assured me that I would not be on a mind numbing amount of opiates. She of course refused the lidocaine to me, but did NOT tell me to stop using it, or discourage it in anyway, which is very good, we discussed nerve pain medications such as gabapentin, and I told her I've used that in the past for other painful random twitches, and it worked for that, but that's it. So no gabapentin.
She did however try me on duloxetine, 30mg a day for a week, then 60mg a day for two weeks, then see how it helps from there, and I checked it out, and I can still take my 5-HTP daily, just watching intake, and monitoring for signs of SS. Though, SHE didn't tell me that, despite my telling her I am on it. Whatever though.
All in all, I feel better about the whole thing now I guess, just have to be patient and wait for my amputation shit to go through, and in the mean time, she says PMT is usually injections, and I am thinking it's injections of lidocaine similar drugs every week or two, and I seem to remember reading somewhere about histamine injections being used, as well as certain steroid injections.
We'll see, I'm willing to try anything. The duloxetine being an SSNRI kinda goes against my whole ANTI antidepressant drug soapbox, but it's not for depression, but neuropathic pain, and it has been proven in double blind studies to help with that type of pain, in NON MDBP patients as compared to placebo, so the possible benefits outweigh the possible risks.
And she gave me a shit ton more hydrocodone.
Also, if they have vials of lidocaine sitting around PMT when I go, I am robbing those bitches blind. mmmgimme some lido with epi, marcaine, ropivacaine, SOMETHING NIGGER.

I'm sorry but I would rather have my health care in the hands of corporations rather than another inefficient government agency. You do need to apply for Medicare, if you had already applied and failed for some bullshit reason then your argument would be more valid to me.
Why do you have a shitty job? Should all the hard working tax payers have to contribute more every year to support you and your problems with overdosing? Just because the majority of Americans don't have insurance doesn't mean they can't receive health care from non profit services or visit clinics. It is illegal to refuse emergency medical service just because you don't have insurance.
I know I am lucky to be covered and my judgment may even be clouded if I was in a similar bad position. I can keep going but I will end on this for now, why do most doctors labor through medical school? Money. Do you think the government is inventing new medicines? No, most innovative research comes from the private sector.
Edit:
yeah consciouscowboy. you're a fucking RETARD!

Re: Pain management therapy
Dude AC most of the nerves are fucked up, what isn't already numb hurts, and it's inside the ankle and shit. But the real point of amputation is because they said after seven years the ankle bones would fuse together, limiting my range of motion even more, causing even more pain. And because of the changed gait that I have because of having to walk on a dead limp foot, with no control, it goes all over, and the rest of my body has to compensate, which fucking HURTS more than anything. It is literally causing damage to healthy parts of my body. My knees, hips, back. It's hard for me to even get out of bed, walk to the bathroom, or any normal everyday shit. Honestly, sometimes I sit down to piss just because it's easier. I am sitting down every single chance I get, because even just standing there fucking KILLS ME.
And of course with all of this comes bouts with depression, hopelessness, ANGER, self loathing, suicidal thoughts, (i.e. people would be better off without having to take care of me), but with good pot, some 5HTP, maybe a few hydros, and my wife consoling me, I do ok on the emotional aspect of it. Much better than I used to. I used to just drink my face off, and I almost did the other night, but I fought it, and won, and am very proud of myself for it. Alcohol has been the only drug besides cigarettes that I have had a problem with, and a cigarette problem isn't bad, but a drinking problem can get quite bad, worse than opiates or any speed, IMO.

I'm sorry to hear that shit man.. have you thought maybe there's a possibility of getting a massive sum of money from a lawsuit? you did say it happened at work.. i mean you got fucked up bad because of the fault of someone else.. you should be entitled to huge man.. as far as it getting worse in the future you're right it might, but instead of thinking about that think how the internet and unlimited access to everything for nearly everyone is going to change the rate of technological advances (medical in particular).. just dont let it get to you and who knows maybe a breakthrough will make things more like before, and maybe even better since you wouldn't be taking it for granted..

so i jacked the vial of the lido, its 10mg/ml 20ml bottle... its about 1/3 of the way full... so by eyeballing it i would estimate 60-70mg of lido still in there if i ever find a way to get it to you, if you would even want that much...

Re: Pain management therapy
I feel bad stupid noob, I was in an out patient operating room a few weeks ago getting a tumor and thorn removed from my forearm (thats another story all together), and they left me alone in the operating room before the doctor came....
Being the drugy and thief that I am, I started looking through everything. Fucking TONS of lidocaine. TONS of the shit. Like at least 1000 bottles but probably more. There was a supply closest with just crates full of lidocaine.
In fact, pretty much the only thing they had in the room was lidocaine, stitch kits, and bandages. I could have easily slipped a hundred vials or so into my winter jacket if I had known you couldn't get it.
I would have traded you for a few ml's of reagent.

Cowboy, yeah I know folks shouldn't have to pay for such things, but when almost all powerful nations have working systems in place for everyone I cannot see why we as Americans cannot as well. An about Medicare, my mother tried when I was 13-14 to get me a card b/c of my diabeties and some other issues and I was denied. But i've never tried since to get some help b/c I feel as long as I can work I shouldn't try to scam a system which ppl really do need to make it, well it actually isn't scamming it I guess.. I dunno man, I just have no insurance and it sucks, thats basically it, I have no real reasoning besides that "it sucks" to be w/o insurance, ya know? Lol

A good friend of mine went to PMT and I think it was somewhere on or off Dixie Highway, I'm not sure. I had never even heard of it when she told me about it. She got barred because on her drug test she tested positive for pot. So I wouldn't go at all, because she claims she hadn't smoked for 2 months. But I'm not sure if she got in any legal trouble, I lost her number.

why bump a 3 month old thread?
dirty urine isn't really a crime unless your already in trouble for something else...
its like, "oh you failed your drug test for this job, sorry..." and "looks like you failed your court mandated drug test while on probation, time for jail"

He bumped it because he is fucking stalking me. On topic though, I never went to PMT. I was five minutes late to my first appt, and they told me too bad, reschedule, and frankly I don't have the time for that shit, not that it would have done any good anyway. I think I will stick with illegal drugs.
