Histamines, Leukotrines, and IV Opiates, oh my!
So what the fuck is up with IV'ing codeine/codeine derivitives? I know that codeine and hydrocodone can potentially cause anaphylaxis, but why not oxycodone too? I know so many people who have shot up oxycodone without any allergenic side effects, so I don't get it, why would it be any different? I'm not planning on banging codeine or hydrocodone but I don't really understand this.

You don't get to call anyone else that. Besides, he did nothing to prove he was an idiot, unlike yourself.
First off let me just say I DO NOT know the answer to the OPs question, but my guess is that codeine has to be converted into morphine before you get any effects. Oxycodone is a different drug entirely and is not converted into morphine. Someone that knows chemistry probably understands this a lot better.

IV codeine causes pulmonary oedema if I recall correctly.
The point about oxy is moot, comparing different opioids in their pharmacokinetics is like comparing different breeds of dog. They're still dogs, but they're nowhere near the same.

In this case it's like some dogs are instantly freindly while others tend to aim for the jugular.
Basically, codeine has to be converted into morphine otherwise it causes massive histamine release, the only place this conversion happens is in the liver, thanks to some enzymes that i couldn't be fucked remembering. :)

AFAIK, leukotrienes have nothing to do with it.
The "allergic" symptoms are all because of histamine, and it has nothing to do with actually being allergic*. Also, oxycodone can cause the same symptoms. All opioids can. There are some people who get itchy as fuck from oxy, and some who don't. Just like there are people who get itchy as fuck from hydrocodone and those who don't. It's all about the individual.
* True and false. It can obviously be a sign of an allergy, but the vast majority of "allergic" reactions to opioids are just innocuous release of histamine. In your body, histamine is stored in cells that, for some reason, can be "triggered" by certain amines to release their stores of histamine. Some opioids just happen to be more likely to cause that release. Here's a chart that shows the relative risk of histamine release by opioid:

Codiene is a prodrug of morphine. Which means it is converted to morphine. (which you understand) Oxycodone is also a prodrug. But it converts to oxymorphone. Hydrocodone converts to hydromorphone.

Codeine is metabolized into norcodeine, morphine, and codeine-6-glucuronide. The norcodeine is further metabolized into normorphine. The morphine is further metabolized into even more normorphine, as well as morphine-3-glucuronide and morphine-6-glucuronide.
Hydrocodone is metabolized into norhydrocodone, hydromorphone, and hydrocodol. The norhydrocodone and hydromorphone are then metabolized to norhydromorphone. The hydromorphone is also metabolized into hydromorphol as is the hydrocodol.
Oxycodone follows the same metabolic pathways as hydrocodone, just replace the words "hydro" above with "oxy" and you've got your chart :p
There's a very good article in an issue of the journal Current Pharmacogenomics from a few years back entitled "Influence of CYP2D6 Genetics on Opioid Kinetics, Metabolism and Response", which is where I got this information. It explains the metabolism of codeine, dihydrocodeine, hydrocodone, oxycodone, and tramadol, including which enzymes do what, and is pretty easy to understand. I'll see if I can find an online copy of it.
