Long story short, i'm on a mission to blackmail someone. I need to get some kind of paperwork for a positive result STD virus test from a doctor's office, & i'll accept any kind of results but of HIV/AIDS. I'm not sure if there's different kinds of paperwork from state to state, & being the one whom it's concerning is in New York state, it would be most relevant to it if it were from there. I don't want any personal information on there, so if it's got a name on there i'll block it out or if it comes blocked out I won't mind that either. I've just got to be able to tinker with it to make it seem it's from a doctor's office, above all else :)

I already did a few searches on it, for if I found it I wouldn't be posting here asking about it. Thank you for any & all help, I will be posting again soon enough!

Generally speaking the format will depend on the clinic/pathology company that did the testing. You should try Bad Ideas... actually, no... most of the people in there have never had sex.

Try one of those STD singles sites. Tell them you received a letter in the mail but you think it's fake, can anyone tell you what a real one looks like etc, maybe scan theirs in (minus personal details).
