It is very possible. If she misses a pill or takes it late in the day or later than she normally does. And there is still a risk.

If the birth control pill, when used pefectly, is 99.9% effective, you have a .01% chance of impregnating the girl the first time you have sex. And the second. And the third. ANd the 5,000th. This is the way to go, although I've never used a diaphragm. Plus, if you 'catch it' early enough getting pregnant isn't the end of the world. You get an abortion pill and move on with your life.
