You need something attached to the ceiling; ideally a pulley wheel, but a fairly wide heavy-gauge hook for hanging heavy potted plants or maybe even a punching bag will work well. Make sure your ceiling is strong enough that it won't pull through since you'll probably have about half your weight on it at times.

You'll need rope, cuffs, a collar, ankle fetters with something to attach them to, a dildo with a base, razorblades or a caustic substance, broken glass (a lightbulb or two is perfect) and a ball gag/dildo gag.

Place the key to your cuffs in a bowl just behind where you'll be sitting. Fill the bowl with razorblades or some caustic substance like strong bleach or oven cleaner. It should be very uncomfortable on your upper arms and shoulders, lower back and the bottom of your legs. The only way to alleviate the pain is to shuffle forwards, which brings your neck and knees up and your wrists and arms down. Unfortunately, you'll be sliding across the broken glass, and it'll be some time before the pain in your body is enough to make you want to do that. You'll get there, though. You have to slide forward enough to get your hands into the bowl (secure it to the ground somehow to stop yourself cheating and knocking/dragging it), and then rummaging around in razorblades for a small, flat object will be hard too. Using some strong acid/alkali which will burn on contact will have the exact same effect. Using the key on the locks behind your back will also be surprisingly tricky when your arms and wrists have been at the initial angle for so long too.

You can find out just how uncomfortable the starting position beforehand is by sitting down with legs flat, bending forwards, clasping your hands behind your back and forcing them upward in that same position as in the drawing. Go ahead, try it. If you want to make it that little more uncomfortable to slide forwards, attach the pulley rope to the back of your gag, which will drag your head back at a pretty nasty angle, but be sure to drag the pulley rope a little tighter if you do to compensate for the backwards movement of the head.
