I usually start with saying hi and introducing myself and then telling them to not get mad if I forget their name. That kinda gets it open because it usually gets you both laughing.

Be unique. It's the dumb jocky boys that ask questions like "so... come here often?" or "what's your favorite position?" Those are dumb.

Ask what her ideal adventure is. Somebody asked me that once, and conversation flowed for 3+ hours... and neither of us even like talking on the phone for more than ten minute.

Everybody asks those questions, and they get boring to answer over and over. The only topics that aren't overplayed are music, drunk stories, and travel.
