Default 99% effective?

Ok, from my memory, the pill is something like 90-98% effective. And a condom is about 70-90% effective. Correct me if im wrong.

But anyways, what would chances be of a girl getting pregnant if she was on the pill, and the guy used a condom? I'm assuming it would be very effectice (but of course, nothing is 100%, besides not doing it at all).

And any other combinations that work really great? Like, condom and a diaphram, etc?

It would be pretty damn effective. Technically not 100% but the chances would be slim enough that you needn't worry much.

Theoretically these methods should be 100% effective. The statistics show otherwise due to "user failure" or variables that can't be known, such as physiological differences in females that might make the pill less effective.

Using more than one method at a time decreases chances of failure; if one method fails, the other will serve as backup. The chances of both methods failing is slim. There really isn't a "magic combo" but common sense was already telling you that a condom plus diaphragm would be a good one.

Depo-provera shots are known to be the most effective, since "user error" is practically eliminated.

Diaphragms are tricky to use and a backup such as a condom should always be used until the female is positive she has learned to insert it properly. The Nuva ring is considered to be as effective as the pill, but doesn't require nearly the precise placement as the diaphragm. But the ring is hormone-based, whereas the diaphragm is not.
