Given that you are flexible enough to reach your genitals with your mouth, not much. If you take a look at yoga, they do positions which are more or less the same if not more twisted up, and they do it for hours. Yoga has no internal stress upon the person given their flexibility extends to the need for said position. (does that make sense?)

So truthfully, not much too bad could happen if you were properly stretched and flexible. You may have to worry about muscle cramping. With respect to organs, nothing. All that would happen is you could end up swallowing your own cum. That wont hurt you. Unless you have genital herpes or warts etc. Then you might want to rethink the idea?
