I knew a guy with an actual third nipple on his chest just below one of his other ones, a third nipple is an abnormality, something useless, so they're saying this guy is undesirable and not worth hanging out with.

i know a guy with a third nip too, although i'm guessing in this context it's like a, "third-wheel," an unwanted tag-along.

From the context it seems like it could mean any of these things, or that he's a general defective person and saying he has a "third nipple" is meant as derogatory speculation. I thought it was kinda funny, though.
